How Do I Grow My Physical Therapy Practice Past $500K Per Year?

Past $500K the work stops being strategy and becomes people and culture. You duplicate what already works with documented systems, weekly scorecards, a clear org chart, and a company vision everyone can see themselves inside of.

01

What has to be in place before I scale?

Every standard operating procedure must be current so the system is plug and play when you open a second location. Most systems get refined between $100K and $500K, so scaling past that is duplication, not invention.

02

How do I build a vision my team buys into?

Build a company-wide vision with a 10-year goal, a 3-year goal, a 1-year goal, and a 90-day goal. Send every team member a Team Vision Survey to learn their ambitions and what motivates them.

Align their dreams with practice milestones. Create an org chart that shows every position's responsibilities and who they report to.

03

How do I hold the team accountable?

Use weekly team scorecards for Google reviews, patient referrals, and new business relationships. Review those scorecards every week, because scorecards without a review process do not create accountability.

Ben's mistake was running scorecards with no review process, so there was no accountability. Fix it with a 30-minute scorecard review and game-plan meeting every week, a 60-minute leadership meeting to review performance and quarterly progress, and one all-hands vision meeting per month.

04

What are the weekly non-negotiables at this stage?

Each physical therapist should reach out to five to 10 or more businesses, have one or two face-to-face meetings with referrers, and run one or two workshops until schedules are full. Each team member should also ask for one or two Google or Yelp reviews and one or two patient referrals.

The team leader or practice manager leads a 30-minute scorecard review, admin completes the marketing profitability tracker, and the team uploads one new ad creative for a split test.

05

What is the real ceiling?

The real ceiling is often what you believe is possible. Ben got stuck at $650K per year because he believed $1M was out of reach, and that belief sabotaged his daily actions.

His fix was proximity: surround yourself with people who have already reached your goal. You grow fastest when you are the smallest person in the room.

06

When is it time for a second location?

It is time for a second location when your SOPs are current, your top performers can lead teams without you, and your time is spent on revenue-generating work instead of putting out fires. Promote from within and let top performers manage the rest of the team.
